Facility Membership(through 2010)
MEMBER
AND BILLING AGREEMENT: By your providing this completed
Facility Application/Renewal Form and USIndoor’s accepting payment
therefor, USIndoor shall recognize the “Facility” named above (or
the named individual above, if no facility is named), as a USIndoor
Facility Member. You or other authorized personnel (“YOU”) may satisfy
payment of Annual Facility Membership Dues (“Dues”) either by mail,
including a check and this completed Form, or by credit card after
clicking “Submit Form” (in either case with payment in U.S. dollars.)
If you click “Submit Form,” you will be transferred to a credit
card payment processor, in which case you hereby represent and warrant
that YOU are duly authorized (1) to make payment by the credit card
to be entered (the “Credit Card”) and (2) to agree to recurrent
charges of Dues by USIndoor. Accordingly, USIndoor shall have the
right to apply an annual charge for Dues against the Credit Card
(whose billing information YOU may change with timely notice) on
or about each anniversary date of the current transaction until
cancelation by YOU by e-mail. Each annual charge shall be the prevailing
Dues at the time, as published by USIndoor. To cancel or change
billing information, please notify USIndoor at admin@usindoor.com
at least thirty (30) days prior to any expected charge. Unless otherwise
notified by YOU, USIndoor shall recognize the e-mail address provided
herein as YOUR official address, where USIndoor may send all billing
communications.
